Clinical Question Do psychosocial interventions reduce the relapse rate in patients with bipolar disorder? Bottom Line Cognitive behavioral therapy and group psychoeducation reduce the risk of relapse in patients with stable bipolar disorder. (LOE = 1a) Study Design Systematic review Funding Government Setting Various (meta-analysis) Synopsis Patients with bipolar disorder may experience frequent relapses, even if treated medically. This systematic review identified any randomized or quasirandomized (i.e., assignment by center or day ofweek) studies of psychosocial interventions to prevent relapse. The authors identified 12 relevant studies. Interventions included cognitive behavioral therapy (5), psychoeducation (3), family therapy (2), care management (1) and group therapy (1). Most studies had <100 patients, although the study of care management had 441 participants. Cognitive behavioral therapy (OR=0.24, 95% CI: 0.12–0.51) and group psychoeducation (OR=0.16, 95% CI: 0.07–0.4) were both effective at preventing any relapse. They were also effective at preventing manic and depressive relapses measured individually. There was some evidence for the effectiveness of family therapy, group therapy or care management, but the literature base was too limited in size and quality to definitively answer the question.
