The exercise effects on skill-related fitness in adolescents with intellectual disability: A systematic review and meta-analysis

Background: Skill-related fitness, which consists of agility, balance, coordination, speed, reaction time, and power, is a component of physical fitness with the ability to perform sports or occupations. However, few researches concern the benefits of exercise on skill-related fitness in adolescents with intellectual disability (ID). Purpose: The purpose of this study was to synthesize and determine whether exercise trainings improve skill-related fitness in adolescents with ID. Methods: We followed the Guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) to complete this systematic review and metaanalysis. Studies with level 2 and 3 of the 2011 Oxford CEBM Levels of Evidence in nine electronic databases were searched. Besides, Physiotherapy Evidence Database (PEDro) scale was used to assess the methodological quality of included studies. Statistical meta-analysis using Comprehensive meta-analysis (CMA) software version 2was applied if possibly quantitative data could be pooled. A random effects model was used for statistical pooling of the data. Effect sizes were expressed as Cohen's d and converted to Hedges's g. Results: Eighteen studies were synthesized for systematic review and 14 studies for meta-analysis retrieved from 805 studies. In those 18 studies, 9 were level 2 studies while the other 9 were level 3. The mean PEDro scores was 4.3. The range of Hedges's g was -2.08 to 1.42. Components in skill-related fitness except balance and coordination could be significantly improved by a variety of exercise trainings in adolescents with ID. Conclusion(s): Exercise training is effective to skillrelated fitness except balance and coordination in adolescents with ID according to this systematic review and meta-analysis. However, this study found that limited studies with low quality led to cautious application. We need more studies with high level of evidence in the future. Implications: Exercise training for adolescents with ID should be addressed to improve the skill-related fitness in special education. Several exercise may be provided to improve the performance of skill-related fitness in adolescents with ID.
